What type of fuel filter do you use? Do you use an second fuel filter system? Do you use any additives for lubricity?
In my opinion you shouldn't have to do all these things after buying a 45 K truck just to get a normal lifespan out of injectors, however I think thats what we need to do. Look at major manufactures life CAT, they use great filters, 1 or 2 micron filters. I think GM just dropped the ball completely on the fuel filters on our truck and thats why the injectors fail so much. The dry low sulpher diesel isn't helping either. Bosch recommends anywhere from 3-5 micron filtration with 97% efficiency. I'm not exactly sure what most fuel filters micron rating is for our trucks, but I'm going to call a couple dealerships- or just drop by and get the PN of the filter that they sell, then I'll be getting ahold of the manufacturer to see what the filtration specs are- and maybe, just maybe we might have a case against GM for not fulfilling their part of the deal. So, in the mean time, I will use I'd use a 2 micron filter that goes on the factory location, I just found this site that has a major name brand 2 mic for cheap
Shop and I'd also run a secondary filter system that can be had for anywhere from 100 to 700$ and lastly I'd run a lubricity additive with every tank. Here is a link for a study that was done in regards to lubricity- I just ordered the opti-lube XPD because it ranked so well in the test.

Hope this helps, I know my truck is in the shop right now getting the injectors replaced and when it gets out I'll have the new 2 micron filter installed before it ever runs on the new injectors, and the day it gets out I'm putting on a remote filter, and using additive every tank to see if it was in fact GM's poor filter design- and because I simply cant afford 4500$ a pop.
