I don't know I've been told by some that if you drill the passenger side the same as the drivers side that you stand a good chance of losing rail pressure faster. Thats what I read over at DP

Yes. Same size is fine. This was one of my first steps back in early 05 in flowing more fuel. The reason some say you lose rail pressure is the placement of the sensor. It's on the junction block on the LB7s (pressure is backed up in the feed lines) instead of the rail like the LLY and newer. It gives you more of an actual reading now that there is no backed up pressure.

How much does a moded regulator valve run? Also how much do the rail fittings run?
