have you tried to use a mechanic stethoscope (dirt cheap at Oreilly's) or a broom handle/long screw driver to get a little closer? If it is ratting that loud, you should be able to get pretty close to it using one of these methods....

The ratteling could be a broken flywheel, Im not sure on the surging part.

check the r harmonic balancer if the rubber is bad on the balancer it will rattle bad when idiling and when you let off the thorottle and will vibrate thru the crank making it sound like the torque converter

my only experience with rattles to this nature was either a spun bearing or the flex plate was shreaded! none of it is cheap!

i had a similar rattle on mine (glad to see someone with more miles than me on an 06 LLY, 95k here)

funny thing is it would go away when I would go thru a car wash... i found that i had 2 things, first was a heat shield near the down pipe that i think was too close to the exhaust... i bent it slightly back away from exhaust... second, i still have my cat & from hitting the exhaust with my hand, it sounds like something is loose rattling in the cat... haven't had the noise for a couple of weeks (& i haven't washed it in a couple of weeks)

So far nothing has worked. Still a mystery to what the rattle is, but there is a new symptom...yippee. I stop the truck and put it into park, as I let off the brakes it rattles then stops. Its starting tick me off, dealer can't find anything either.
