I had that issue. Roughly 2 gallons spilled out. It was the pump it self. It was pumping so slow it didn't foam up and stop the pump. I wouldn't worry about it. Just avoid that pump next time.

Def the fuel stations it always seems like they skip over the maintenance on their diesel pumps. Try going to a truck stop where the pumps get used all the times they seem to always have the best pumps. I just filled up today and I always just take my time because of tr foam. If you just let it run till it stops chances are its not completely full anyway
