A friend of mine has the same 2016 2500 Sierra with the Duramax and about 30,000 miles, and last week his DEF heater went out. He asked about mine and I told him that I just put a box of DEF in it. Well yesterday my engin lite came on. I checked the code and it was a P21DD, DEF heater low voltage. I'm wondering if it has to do with age and the corrosive nature of the DEF, and the now low temps. He stated that the Chevy dealers are still warranting the DEF heaters and not charging for the work. I will try our local GMC dealer where the truck was purchased.
Has anyone else noticed that there are a lot more DEF heater issues with the colder weather?

My 2011 I believe has had 3 GM POS's heaters replaced. Since it is beyond the extended warranty I bought I think a better one from the DMAX store for when it fails again. Made by Bosch I believe. Yeah seems like when it got cold is when they failed. .
